Description
Plantar anatomy of the male midfoot is centered on the ligamentum cuneocuboideum plantare (plantar cuneocuboid ligament), a stout band spanning from the plantar surface of the medial cuneiform toward the plantar aspect of the cuboid. Deep to it lie the tarsometatarsal and transverse tarsal joint complexes, with the cuboid positioned lateral to the cuneiforms and the navicular seated proximomedially, while the bases of metatarsals I to V extend distally from the tarsus. Adjacent plantar supports that are typically appreciated in the same field include the long plantar ligament (calcaneus to cuboid and metatarsal bases) and the plantar calcaneonavicular (spring) ligament medially, which together contour the midfoot and forefoot arches. Orientation matters here, because medial and lateral columns can be compared at a glance. Midfoot pain and instability often come down to the integrity of the transverse arch and the lateral column, and the plantar cuneocuboid fibers are part of that story. Surgeons working around the tarsometatarsal joints and the calcaneocuboid region (for example during lateral column lengthening, midfoot arthrodesis, or after Chopart and Lisfranc injury patterns) use this ligament as a reference when assessing plantar restraint and joint congruity. Small structure, big consequence. When the plantar ligamentous complex fails, diastasis and collapse can persist even after dorsal fixation.
