Description
Dorsal cuneonavicular ligaments span the dorsal aspect of the midfoot, bridging the navicular bone proximally to the medial, intermediate, and lateral cuneiforms distally. Multiple short fibrous slips course in a generally anterodistal direction from the dorsal navicular toward each cuneiform, lying deep to the expected path of extensor tendons across the dorsum of the foot. Medial and lateral margins of the navicular and the dorsal surfaces of the cuneiforms define the attachment footprint, with the tarsometatarsal region and metatarsal bases positioned more distally in the same superior (dorsal) view. Small bands, big consequences. This dorsal perspective matters because the cuneonavicular complex forms part of the transverse tarsal (midtarsal) region and helps constrain dorsal gapping at the naviculocuneiform joints during gait, when the medial column transitions from mobile to rigid. Dorsal midfoot sprains, midfoot instability, and post traumatic arthrosis often localize to these short dorsal bands, and surgeons planning dorsal approaches for naviculocuneiform arthrodesis or ORIF rely on these landmarks to anticipate capsuloligamentous disruption and protect the overlying dorsalis pedis artery and deep fibular (peroneal) nerve. The clustered, segmental insertions also explain why partial injuries can be clinically subtle yet mechanically relevant.
