Description
Dorsal soft tissues have been cleared to expose the tarsometatarsal (Lisfranc) region, with short dorsal tarsometatarsal ligaments spanning from the distal tarsal row to the bases of the five metatarsals. The cuneiforms and cuboid lie proximal to the metatarsal bases, while the phalanges extend distally, preserving the longitudinal alignment of each ray. Proximally, the talus and calcaneus frame the hindfoot, but the visual emphasis stays on the dorsal ligamentous bands crossing the midfoot joints in an anteroposterior direction. The dorsal capsule and ligament fibers sit superficial to the articular margins, forming a thin restraint compared with the deeper interosseous stabilizers. Dorsal tarsometatarsal ligaments matter because dorsal failure is a common component of Lisfranc injury patterns, where diastasis between the first and second metatarsal bases and malalignment of the medial and middle columns can be subtle on early radiographs. This view supports teaching why plantar and interosseous structures often remain the stronger restraints, while the dorsal ligaments can rupture with low energy twisting mechanisms, producing dorsal instability and midfoot pain on weight bearing. A clean dorsal perspective also helps when discussing surgical incisions for open reduction and internal fixation across the first through third tarsometatarsal joints, where dorsal neurovascular structures and extensor tendons must be respected.
