Description
Running obliquely within the sinus tarsi, the cervical ligament (ligamentum cervicis) spans from the superior aspect of the calcaneus to the neck of the talus, forming a stout talocalcaneal band deep to the lateral hindfoot. A medial view with the hallux oriented to the left places the talus superior to the calcaneus, with the tarsal canal and sinus tarsi lying between them, while the metatarsals and phalanges extend distally. Adjacent stabilizers of the subtalar complex, including components of the interosseous talocalcaneal ligament and surrounding capsuloligamentous tissue, sit in close proximity and are typically rendered as contrasting white and blue connective cords against the smooth tarsal bones. For teaching subtalar mechanics, this ligament is a clean landmark because it checks excessive inversion and contributes to coupling between talar rotation and calcaneal motion during gait. Injury patterns cluster here. Lateral ankle sprain with persistent hindfoot pain can involve subtalar instability or sinus tarsi syndrome, where disruption or scarring around the cervical ligament and interosseous talocalcaneal ligament becomes clinically relevant during examination, arthroscopy of the subtalar joint, or targeted rehabilitation.
