Description
Running adjacent to the midline, the paired interspinales muscles span between successive lumbar spinous processes, with short fleshy bellies and small tendinous attachments concentrated posterior to the vertebral laminae. Lumbar vertebral bodies sit anteriorly, separated by intervertebral discs, while the spinous processes project posteriorly into the interspinous spaces that the interspinales occupy. Small slips lie medial to the longer erector spinae columns, and they blend locally with the thoracolumbar fascia and the interspinous ligament. Scale matters here. Clinically, this posterior lumbar perspective helps you teach why the interspinales are classified among the deep intrinsic back muscles despite their modest cross sectional area: they act as segmental extensors and, more importantly, as local stabilizers and proprioceptive organs at the interspinous interval. Their close relationship to the interspinous ligament and adjacent multifidus makes them relevant in discussions of midline posterior approaches, where dissection or retraction around the spinous processes can denervate or strip these short fascicles. The same anatomical neighborhood is targeted in interspinous process device placement and is a pain generator in Baastrup disease (kissing spines), where hypertrophy and bursal change occur between adjacent spinous processes.
