Running along the anterior surfaces of the lumbar vertebral bodies, the anterior longitudinal ligament forms a continuous fibrous band that bridges the intervertebral discs from superior to inferior, hugging the midline and widening slightly over each disc space. Lateral to the vertebral column, the deepest spinal musculature appears as paired longitudinal masses, with fascicles oriented craniocaudally and separated medially by the vertebral bodies and discs. Intervertebral discs sit between adjacent vertebrae as pale fibrocartilaginous cushions, while the bony landmarks of the lumbar spine provide the rigid scaffold for these soft tissue attachments. Orientation is clear. That anterior ligament matters clinically because it limits hyperextension and helps contain anterior disc material, so its relationship to the disc and vertebral body is central when teaching annular tears and patterns of disc herniation. In anterior approaches to the lumbar spine (for example, ALIF), surgeons work in close proximity to the anterior longitudinal ligament during discectomy and implant placement, and its integrity influences segmental stability after reconstruction. Degenerative disc disease, osteophyte formation along the vertebral body margins, and traumatic hyperextension injuries all read differently when you keep this anterior band in mind.
