Description
Posterior thoracic spine and rib cage fill the frame, with multiple thoracic vertebrae stacked superior to inferior and paired ribs sweeping laterally to form the thoracic cage. Between adjacent vertebral bodies, intervertebral discs sit centrally, while dense longitudinal ligamentous bands track cranio-caudally along the column and blend at segmental levels near the costovertebral articulations. Intercostal muscles occupy the intercostal spaces, spanning from the inferior border of each rib to the superior border of the rib below and running obliquely along the thoracic wall. Costovertebral and costotransverse joints anchor each rib posteriorly to the vertebral bodies and transverse processes. This combination of ribs, discs, and ligamentous restraints is the mechanical core of thoracic stability, and it is where clinicians localize pain generators in thoracic spondylosis and posterior rib dysfunction. Small changes in disc height or capsuloligamentous stiffness at the costovertebral joints can alter rib excursion and contribute to painful respiration, a common complaint after minor trauma or in inflammatory arthropathies. It also maps cleanly onto procedural anatomy, since thoracic paravertebral blocks and costotransverse approaches rely on accurate understanding of posterior rib attachments and adjacent soft tissue planes.
