Description

Running along the thoracic vertebral column, the longitudinal spinal ligaments appear as fibrous blue bands aligned with the stacked vertebral bodies and the intervening intervertebral discs. An anterior band hugs the ventral surfaces of the vertebral bodies and discs (anterior longitudinal ligament), while a posterior band tracks within the vertebral canal along the posterior aspect of the bodies (posterior longitudinal ligament). Posterior to the canal, the spinous processes project dorsally, and the facet joints sit posterolaterally, framing the intervertebral foramina where segmental spinal nerves exit. Thickness and fiber orientation in these ligaments are not uniform from cervical to lumbar levels, and the thoracic region is the inflection point where rib-bearing vertebrae, relatively coronal facet joint planes, and a narrower canal change the mechanical demands on the backbone. Clinically, the posterior longitudinal ligament is a key structure in posterior disc herniation patterns and in ossification of the posterior longitudinal ligament (OPLL), where progressive calcification can narrow the canal and compress the spinal cord, even when disc height appears preserved. The anterior longitudinal ligament, by contrast, is often implicated in hyperextension injury and in anterior osteophyte formation, influencing esophageal symptoms in higher segments and surgical exposure during anterior approaches. Educators can drop this plate directly into thoracic spine teaching for gross anatomy, orthopedics, and neurosurgery modules when clarifying how vertebral bodies, discs, facet joints, and foramina relate to stabilizing connective tissue bands.
