Description
Seen in lateral profile, the translucent skin layer reveals the platysma draped over the anterolateral neck, with the sternocleidomastoid running obliquely from the mastoid process to the manubrium and medial clavicle. Superiorly, the temporalis and masseter frame the zygomatic arch and mandibular ramus, while posteriorly the trapezius and deeper splenius capitis contour the nuchal region adjacent to the cervical vertebrae. Between these muscle masses, the anterior and middle scalene align with the cervical transverse processes, and the ligamentum nuchae tracks along the posterior midline as a fibrous anchor for cervical extensor attachments. Orientation is clear. This lateral neck view matters because it is the map clinicians use at the bedside: the sternocleidomastoid divides the neck into anterior and posterior triangles, guiding palpation of cervical lymph nodes and surface location of the carotid sheath deep to its anterior border. An accessory nerve (CN XI) injury is classically discussed in relation to posterior triangle dissections, where it runs superficially before entering trapezius, and this muscle layering explains why shoulder droop can follow node biopsy. Torticollis and cervical dystonia also make more sense when you can see how unilateral sternocleidomastoid shortening rotates the face contralaterally and tilts the head ipsilaterally, with compensatory tension along the upper trapezius and suboccipital region. Use this asset in gross anatomy and head and neck modules to teach surface anatomy, triangle boundaries, and relationships of the cervical musculature to the mandible, mastoid, and upper cervical spine. It also reads well in ENT and endocrine surgery texts when introducing incision planning and safe dissection planes in the lateral neck.
