Description
Seen in lateral profile beneath semi-transparent skin, the superior obliquus capitis spans from the transverse process of the atlas (C1) to the lateral portion of the inferior nuchal line of the occipital bone, lying deep to the posterior cervical musculature at the suboccipital region. The muscle sits posterior to the atlanto-occipital joint and just superior to the posterior arch of C1, with the axis (C2) and the atlantoaxial complex positioned immediately inferior. Adjacent landmarks include the occipital squama superiorly, the mastoid region laterally, and the upper cervical spinous processes and laminae posteriorly. Orientation is clear. This lateral perspective matters because the superior obliquus capitis is a key landmark for understanding fine control of head extension and ipsilateral rotation at the upper cervical spine, where small positional errors translate into disproportionate symptoms.
