Centered in the suboccipital region, the superior obliquus capitis (obliquus capitis superior) runs superomedially from the transverse process of the atlas (C1) to the lateral part of the inferior nuchal line on the occipital bone. Superficial layers have been removed to expose the posterior neck musculature, so the splenius capitis and semispinalis capitis lie posterior to deeper suboccipital fibers, while the occipital belly of the occipitofrontalis occupies the posterior scalp superior to the nuchal lines. Lateral to the midline, the sternocleidomastoid contours the posterolateral neck, framing the deeper attachments at the craniovertebral junction. Fiber direction is clear. For teaching the craniovertebral junction, a posterior exposure clarifies how the superior oblique contributes to extension and ipsilateral lateral flexion at the atlanto-occipital articulation while coordinating with rectus capitis posterior major and minor and the inferior oblique around C1 to C2. Its position also anchors discussion of the suboccipital triangle, where the vertebral artery sweeps posteriorly in the groove of the atlas before entering the foramen magnum, and where the suboccipital nerve (dorsal ramus of C1) emerges to supply these muscles. That anatomy matters in cervicogenic headache and occipital pain patterns, and it is a practical map for posterior approaches that dissect through semispinalis capitis to reach the posterior arch of C1 during C1 laminectomy or occipitocervical instrumentation.
