Description
Centered in the suboccipital region, the obliquus capitis superior (superior oblique capitis) runs superomedially from the transverse process of the atlas (C1) to the lateral portion of the inferior nuchal line of the occipital bone. Superficial layers include the occipital belly of occipitofrontalis over the posterior scalp and, more inferiorly, trapezius and splenius capitis spanning the posterior neck, with sternocleidomastoid and the scalene group framing the lateral cervical contour. Deep to these, semispinalis capitis occupies the paramedian posterior neck, while the highlighted superior oblique traces an angled course just lateral to the midline and superior to the C1 posterior arch. Small but specific. Suboccipital anatomy rewards a posterior view because small changes in fiber direction correspond to distinct rotational and extension vectors at the atlanto-occipital joint, and the obliquus capitis superior forms the superolateral boundary of the suboccipital triangle alongside rectus capitis posterior major and obliquus capitis inferior. That triangle is a surgical and injection landmark, with the suboccipital nerve (dorsal ramus of C1) traversing the region and the vertebral artery coursing in the groove on the posterior arch of C1 before entering the foramen magnum. Clinically, hypertone or strain in these deep extensors is a common contributor to cervicogenic headache patterns and post whiplash pain, and accurate localization helps distinguish myofascial trigger points from greater occipital nerve irritation.
