Description
Centered in the suboccipital region, the obliquus capitis superior is presented bilaterally deep to the posterior cervical surface tissues in a male, with its fibers running superomedially from the transverse process of the atlas (C1) to the occipital bone between the superior and inferior nuchal lines. Superficial layers include the occipitalis over the posterior scalp and the upper trapezius over the nuchal ligament, while deeper extensors such as splenius capitis and semispinalis capitis are reflected to expose the short suboccipital muscles. Medial to the obliquus capitis superior, the rectus capitis posterior major and minor occupy the midline suboccipital compartment, and inferiorly the obliquus capitis inferior courses from C2 to C1, framing the suboccipital triangle. Small muscles matter here. This posterior exposure is the teaching sweet spot for understanding fine control of atlanto-occipital motion and the neurovascular crowding that makes the area clinically unforgiving. The suboccipital nerve (dorsal ramus of C1) runs in close relationship to these muscles, and the vertebral artery arches across the posterior arch of C1 before entering the foramen magnum, a relationship surgeons must respect during posterior C1 lateral mass instrumentation or suboccipital decompression. Myofascial tenderness in the suboccipital group, including the obliquus capitis superior and rectus capitis posterior minor, is often discussed in cervicogenic headache patterns and can be confused with greater occipital neuralgia, which typically involves the C2 dorsal ramus as it pierces semispinalis capitis more inferiorly.
