Description
Posteriorly, the occipital bone and its inferior nuchal region sit superior to the upper cervical spine, with the atlas (C1) immediately inferior to the craniovertebral junction and the axis (C2) deep to it. The superior obliquus capitis lies in the suboccipital compartment, running superomedially from the transverse process of C1 to the occipital bone lateral to the midline, positioned lateral to rectus capitis posterior major and deep to the skin and superficial posterior neck musculature. Nearby landmarks include the mastoid process of the temporal bone laterally and the external ear as surface context. Orientation of obliquus capitis superior matters because it frames the functional mechanics of the atlanto-occipital and upper cervical complex, where fine control of head extension and ipsilateral rotation couples with stabilization rather than gross power. This posterior exposure is also the cleanest teaching view for the suboccipital triangle region, where the vertebral artery and the suboccipital nerve (dorsal ramus of C1) course deep to these muscles and can be irritated in cervicogenic headache patterns or during posterior C1 instrumentation.
