Description
Arising from the transverse process of the atlas (C1), the superior obliquus capitis courses superomedially to insert on the occipital bone between the inferior nuchal line and the area lateral to the foramen magnum. Seen from a posterior, subcutaneous perspective in a human male, the paired muscle bellies sit deep in the upper neck, lateral to the midline occipital crest and superior to the posterior arch of C1, with the axis (C2) and the remaining cervical spinous processes descending inferiorly. Portions of the scapulae frame the lower field, reinforcing the cervicothoracic alignment and the midline orientation of the cervical spine. Small neighboring suboccipital muscles may also be visible near the occiput, including rectus capitis posterior minor and major, tucked closer to the midline. This posterior view matters because the superior obliquus capitis forms the superolateral boundary of the suboccipital triangle, a compact corridor that transmits the vertebral artery (V3 segment) and the suboccipital nerve (dorsal ramus of C1) as they curve across the posterior arch of the atlas. It is a tight space. In posterior C1-C2 instrumentation, exposure and retraction in this region must respect the vertebral artery’s course and the nearby atlanto-occipital and atlantoaxial joints, while clinicians evaluating cervicogenic headache or occipital pain often localize tenderness and trigger points in the suboccipital musculature just inferior to the occipital bone.
