Description
Seen from the lateral aspect of the hindfoot, the lateral talocalcaneal ligament spans a short course between the lateral surface of the talus and the superolateral calcaneus, deep to the fibular malleolus and the more superficial calcaneofibular ligament. Superiorly, the distal fibula and lateral malleolus frame the ankle mortise over the talar trochlea, while inferior to the talus the calcaneus forms the floor of the subtalar joint. Anteriorly, the tarsal contour continues toward the cuboid and metatarsals, with the toes projecting distally. Stability at the subtalar joint depends on small, stout bands as much as the larger ankle ligaments, and the lateral talocalcaneal ligament is a consistent restraint to excessive inversion and subtalar tilt. This is the ligament you want in mind when lateral hindfoot pain persists after an ankle sprain, or when stress radiographs and CT suggest subtalar instability despite an intact anterior talofibular and calcaneofibular ligament complex. For surgeons, its deep position makes it relevant during lateral approaches to the subtalar joint and during sinus tarsi and hindfoot arthroscopy, where distinguishing capsular tissue from true talocalcaneal fibers affects debridement and reconstruction decisions.
