Description
Posteriorly, the medial ankle is opened to highlight the medial tibiocalcaneal ligament as a broad band descending from the posterior colliculus of the medial malleolus of the tibia toward the medial surface of the calcaneus. Adjacent osseous landmarks include the distal tibia and fibula superiorly, with the trochlea of the talus seated between the malleoli inferiorly; the posterior aspect of the tibiotalar joint capsule is suggested by bluish capsular or articular surface treatment. Medially, the ligament fibers run inferiorly and slightly anteriorly, occupying the deep plane where the deltoid ligament complex reinforces the talocrural joint. A stabilizing sling. This posterior perspective matters when teaching medial ankle stability beyond the more frequently discussed anterior talofibular and calcaneofibular ligaments. The tibiocalcaneal component of the deltoid ligament resists valgus tilt and limits excessive eversion, and it becomes a key structure in “high-grade” deltoid injuries associated with pronation-external rotation mechanisms and some ankle fractures that widen the medial clear space. Surgeons repairing deltoid disruption or assessing syndesmotic fixation often reference the medial malleolus and deep deltoid footprint, and this angle helps clarify why posterior-medial tenderness can indicate deep ligament compromise rather than a superficial sprain.
