Description
Medial anatomy of the hindfoot is centered on the talus seated superior to the calcaneus, with the distal tibia forming the roof of the ankle mortise and the medial malleolus projecting distally on the medial side. A distinct medial talocalcaneal ligament band spans between the posteromedial talus and the sustentaculum tali of the calcaneus, crossing the medial aspect of the subtalar joint just inferior to the ankle joint line. Anteriorly, the navicular and the medial column of the midfoot extend toward the metatarsals and phalanges, while articular cartilage is indicated along the tibiotalar and talocalcaneal surfaces. Clear boundaries matter here. Orientation of this ligament is clinically relevant when teaching subtalar stability and hindfoot alignment, because restraint on the medial side of the talocalcaneal joint influences inversion and couples with the spring ligament complex beneath the talar head. Injury patterns in severe ankle sprains, hindfoot valgus deformity, and posterior tibial tendon dysfunction are often discussed in terms of medial support failure, and this medial perspective clarifies what lies deep to the medial malleolus and above the sustentaculum tali. Surgeons working medially for subtalar arthrodesis or tarsal tunnel region approaches also need a clean mental map of how ligamentous bands relate to the bony shelf of the sustentaculum.
