Description
Oriented from the medial side, the ankle mortise is formed by the distal tibia and fibula clasping the trochlea of the talus, with the calcaneus positioned inferior and posterior as the hindfoot foundation. Spanning from the medial malleolus to the talus, calcaneus, and navicular, the deltoid ligament complex appears as a dense, fan-shaped band with superficial and deep fibers that resist valgus tilt and external rotation of the talus. Distal to the ankle, medial midfoot supports such as the plantar calcaneonavicular (spring) ligament and components of the bifurcate and intertarsal ligamentous bands can be appreciated in relation to the navicular, cuneiforms, and the bases of the metatarsals. The longitudinal arch is the organizing geometry. Clinically, this medial perspective is the one you want when teaching why posterior tibial tendon dysfunction progresses to acquired flatfoot: loss of dynamic support unmasks insufficiency of the spring ligament and medial stabilizers, allowing the talar head to plantarflex and adduct while the hindfoot drifts into valgus. Deltoid injury also matters in acute ankle trauma, where medial tenderness or widening suggests instability and may accompany fibular fracture or syndesmotic disruption, changing management from simple sprain care to operative fixation.
