Description
Arising from the posterior aspect of the lateral femoral condyle, the plantaris muscle belly lies deep to the lateral head of gastrocnemius and superior to soleus, then quickly tapers into a long, slender tendon that descends along the posterolateral leg. From a lateral perspective, the fibula runs lateral to the tibia, with the plantaris tendon tracking inferiorly toward the ankle, closely paralleling the calcaneal (Achilles) tendon but remaining more superficial and slightly medial as it approaches the hindfoot. The lateral malleolus, talus, and calcaneus define the ankle and subtalar region, while the metatarsals and phalanges extend anteriorly into the forefoot. Bony landmarks stay clear. For clinicians, this lateral relationship between plantaris tendon and the triceps surae complex matters because plantaris rupture can mimic an Achilles injury and presents as acute posteromedial calf pain in so called tennis leg. Surgeons also care about its course when harvesting the plantaris tendon as an autograft, and when interpreting MRI or ultrasound where a thin, low signal tendon can be mistaken for a split tear of the Achilles or a fascial band. The view also clarifies why the plantaris is functionally minor yet anatomically confusing.
