Description
Seen from the lateral aspect, the distal tibia and fibula form the ankle mortise around the talus, with the calcaneus lying inferior and posterior to define the heel and the subtalar joint line. Along the posterolateral malleolar groove of the fibula, the synovial tendon sheaths for fibularis longus and fibularis brevis track distally beneath the superior and inferior fibular retinacula, then curve anteriorly toward their insertions. More anteriorly, the lateral ligaments of the ankle, most prominently the anterior talofibular ligament and calcaneofibular ligament, span from the fibula to the talus and calcaneus, while the tarsal and metatarsal bones align distally toward the phalanges. A lateral view of tendon sheaths is where peroneal pathology makes sense anatomically. Stenosing tenosynovitis, intrasheath subluxation, and longitudinal split tears of fibularis brevis cluster at the retromalleolar segment, where the tendons share a confined synovial space and rely on the fibular retinacula for restraint. It is also a clean teaching view for distinguishing intra-articular ankle sprain patterns (ATFL injury in plantarflexion inversion) from extra-articular lateral hindfoot pain driven by peroneal tendon sheath inflammation. Small spaces, predictable symptoms. Use this plate in gross anatomy and musculoskeletal ultrasound teaching to orient the probe over the lateral malleolus and trace the peroneal tendons within their sheath, or in orthopedic and sports medicine texts discussing lateral ankle instability and peroneal tendon disorders.
