Description
Seen from the medial aspect of the male ankle and foot, the distal tibia and medial malleolus sit superior to the talus, with the calcaneus positioned posterior and inferior at the heel. Along the posteromedial border, the flexor retinaculum spans from the medial malleolus toward the calcaneus and forms the roof of the tarsal tunnel, where the synovial tendon sheaths of tibialis posterior, flexor digitorum longus, and flexor hallucis longus course in close, layered succession. More distally, the navicular, medial cuneiform, and first metatarsal help frame the medial longitudinal arch, while fibrous and synovial sheath components are emphasized around the tendons as they turn toward their insertions. Tight anatomy. Little margin for swelling. That arrangement matters because tendon sheath friction and tunnel crowding drive common posteromedial ankle complaints. Tenosynovitis of tibialis posterior or flexor hallucis longus can thicken the synovial lining and increase pressure beneath the flexor retinaculum, a setup that often overlaps clinically with tarsal tunnel syndrome affecting the tibial nerve. The medial view also clarifies why stage II posterior tibial tendon dysfunction collapses the arch, the tendon’s line of pull and its relationship to the navicular tuberosity are obvious in this projection.
