Description
Across the anterior ankle and dorsum of the male foot, the extensor tendons are shown enclosed by their synovial tendon sheaths as they pass deep to the superior and inferior extensor retinacula and continue distally toward the toes. Medially, the tibialis anterior tendon tracks toward the medial cuneiform and base of the first metatarsal, while the extensor hallucis longus runs more centrally to the distal phalanx of the hallux. Lateral to these, extensor digitorum longus slips diverge toward digits two through five, with fibularis tertius reaching the base of the fifth metatarsal; the underlying talus, calcaneus, tarsals, metatarsals, and phalanges provide clear osseous landmarks for orientation. Small sheaths, big consequences. Emphasizing tendon sheaths in an anterior view matters because friction control at the ankle is not theoretical, it is where extensor tendon tenosynovitis and retinacular irritation localize clinically and where swelling can produce pain with resisted dorsiflexion and toe extension.
