Beneath the palmar skin of a left male hand, the common flexor tendon sheath is rendered as a synovial sac enveloping the flexor digitorum superficialis and flexor digitorum profundus tendons as they course distally into the central palm and toward the digits. Proximally, the sheath continues into the carpal tunnel deep to the flexor retinaculum (transverse carpal ligament), where it lies anterior to the carpal bones and in close company with the median nerve. Radially, the flexor pollicis longus tendon and its synovial sheath (radial bursa) sit lateral to the common flexor sheath (ulnar bursa), while the palmar aponeurosis spans superficially over the tendon planes. Color separation of muscle bellies, tendons, and retaining bands makes the layered anatomy legible. Clinically, the continuity between the ulnar bursa and digital synovial sheaths explains classic routes for spread of pyogenic flexor tenosynovitis, including potential communication toward the thumb via the radial bursa to form a horseshoe abscess in the deep palmar space.
