Description
Running from the distal forearm across the carpus into the palm, the flexor pollicis longus tendon is enclosed by its synovial sheath (vagina synovialis tendinis) as it courses deep to the flexor retinaculum and continues distally toward the thumb. An anterior (palmar) relationship places the tendon and sheath superficial to the first metacarpal and phalanges of digit I but deep to the palmar aponeurosis, while remaining lateral to the flexor digitorum tendons in the central palm. Proximally, the sheath tracks toward the radial side of the carpal tunnel, then follows the tendon to its insertion on the base of the distal phalanx of the thumb. Clean anatomy, clear pathway. Clinically, this tendon sheath matters because it is the route along which infectious flexor tenosynovitis can spread, and in the thumb it may communicate proximally with the radial bursa, allowing extension into the carpal tunnel region. Surgeons also use these relationships during A1 pulley release for trigger thumb and when planning approaches that must protect the median nerve in the carpal tunnel and the radial artery as it passes to the deep palmar arch. Teaching the sheath as a true synovial-lined tunnel helps explain why inflammatory synovitis produces painful, constrained thumb flexion and why steroid injection placement must respect the tendon’s course.
