Description
Palmar wrist and hand anatomy is rendered from an anterior (volar) perspective, centered on the tendon sheath (vagina synovialis) surrounding the common flexor tendons as they pass deep to the flexor retinaculum into the carpal tunnel. Superficial to this tunnel, the palmar aponeurosis fans distally toward the digital rays, while the metacarpals and phalanges provide bony reference points for the course of the flexor digitorum superficialis and flexor digitorum profundus tendons toward the digits. Proximally the sheath expands in the carpal region, and distally it continues toward the fibrous digital sheaths, maintaining the tendons in close apposition to the phalanges. A tight space. Clinical relevance sits squarely in the mechanics and pathology of the carpal tunnel: thickening of the synovial sheath (flexor tenosynovitis), fluid within the ulnar and radial bursae, or space-occupying lesions can increase tunnel pressure and compress the median nerve.
