Description

Arising from the medial process of the calcaneal tuberosity and the flexor retinaculum, the abductor hallucis runs anteriorly along the medial border of the plantar foot, forming the superficial muscular ridge that frames the medial longitudinal arch. Its belly lies medial to the flexor digitorum brevis and inferior to the talus and navicular region, then tapers toward insertion on the medial side of the base of the proximal phalanx of the hallux via the medial sesamoid complex. Proximally, the tibia descends into the medial malleolus, with the calcaneus positioned inferior and posterior, and the Achilles tendon inserting on the posterior calcaneal tuberosity. Under the skin, the plantar aponeurosis and adjacent fascia contour the sole. Simple landmarks, but easy to misread. Clinically, this medial perspective matters because the abductor hallucis overlies the medial plantar neurovascular bundle and borders the tarsal tunnel region as the tibial nerve and posterior tibial artery pass posterior to the medial malleolus beneath the flexor retinaculum. Hypertrophy, spasm, or postoperative scarring in this compartment can contribute to medial plantar nerve entrapment, and the muscle is routinely referenced when teaching the path of Baxter’s nerve (inferior calcaneal nerve) and medial heel pain patterns. Surgeons also use this surface ridge when planning a medial approach for plantar fascia release or for decompression procedures where maintaining orientation to the calcaneus and navicular is mandatory.
