Description
Oriented to the plantar aspect of the male foot, the abductor hallucis is identified along the medial border, just inferior to the skin and immediately deep to the plantar aponeurosis. Its muscle belly arises from the medial process of the calcaneal tuberosity and runs anteriorly toward the medial sesamoid complex and base of the proximal phalanx of the hallux, lying medial to the flexor digitorum brevis in the first plantar layer. Distally, the abductor hallucis parallels the course of the flexor hallucis longus tendon as it passes toward the hallux, while the medial plantar neurovascular bundle tracks deep to the muscle near the navicular and first metatarsal region. A clear medial landmark. That topographic relationship matters in medial heel pain and nerve entrapment. Hypertrophy or increased tone of abductor hallucis can contribute to compression of the medial plantar nerve (often discussed in jogger’s foot), and the muscle’s proximal attachment sits adjacent to the medial calcaneal tubercle where plantar fasciitis produces focal tenderness. Surgeons also reference this corridor during tarsal tunnel decompression and medial approaches to the first ray, where distinguishing plantar aponeurosis from the underlying abductor hallucis helps prevent iatrogenic injury and orients the dissection toward deeper quadratus plantae and flexor tendons.
