Seen from the plantar aspect, the abductor hallucis occupies the medial sole as a thick intrinsic muscle running anteroinferiorly from the calcaneal tuberosity toward the hallux. Its belly lies medial to the flexor digitorum brevis and parallels the medial longitudinal arch, with the tendon coursing distally to the base of the proximal phalanx of the great toe at the metatarsophalangeal level. Deep and lateral to it, the tarsals and metatarsals form the bony arch framework, while the phalanges of the hallux sit distal and medial as the terminal lever arm. Joint surfaces are indicated along the interphalangeal and metatarsophalangeal articulations. Clear landmarks. For teaching plantar foot biomechanics, this muscle matters because it defines the medial border of the first layer and provides a consistent orientation point for the relationship between the hallux and the arch. Clinically, abductor hallucis is a common site for entrapment of the medial plantar nerve (Jogger’s foot) as the nerve courses deep to the abductor hallucis near the calcaneus, producing medial plantar pain and paresthesia in the great toe region.
