Description

Medial elbow anatomy is presented with the distal humerus articulating with the trochlear notch of the ulna, while the radial head remains partly visible lateral to the ulna at the proximal radioulnar joint. Prominent on the medial side, the ulnar collateral ligament (UCL) spans from the medial epicondyle to the sublime tubercle and medial margin of the coronoid process, with a distinct anterior band anteriorly and a posterior band coursing posteromedially toward the medial olecranon. The fibrous joint capsule blends with these stabilizers around the humeroulnar articulation, and adjacent musculotendinous structures are suggested along the flexor-pronator mass, with brachialis fibers inserting onto the ulnar tuberosity anteriorly. Orientation is clear. A medial perspective matters because valgus stability at the elbow is dominated by the anterior band of the UCL, which becomes the primary restraint from roughly 20 to 120 degrees of flexion, while the posterior band tightens in deeper flexion and relates closely to the cubital tunnel region. Throwing athletes most often fail the anterior band near its distal attachment at the sublime tubercle, the classic substrate for symptomatic valgus instability and reconstruction (Tommy John surgery). This view also supports teaching the relationship between UCL insufficiency and secondary radiocapitellar overload, a common driver of lateral chondral wear despite the medial origin of the problem.
