Description
Oriented from the medial side of a male elbow, the ulnar collateral ligament (UCL) is rendered as a broad, triangular band spanning from the medial epicondyle of the distal humerus to the medial margin of the ulna. Anteriorly, fibers align toward the sublime tubercle at the base of the coronoid process (anterior bundle), while more posterior fibers blend with the posteromedial capsule and approach the medial aspect of the olecranon (posterior bundle). Deep to the ligament, the humeroulnar articulation sits between the trochlea and the trochlear notch, with capsular thickening continuous with the UCL along the medial joint line. Medial elbow stability lives here. A medial perspective is the working view for understanding valgus restraint and the anatomy that matters in throwing injuries. During late cocking and early acceleration, tensile load concentrates along the anterior bundle, the portion most often compromised in overhead athletes and assessed clinically with valgus stress testing. This orientation also supports teaching of surgical UCL reconstruction, where tunnel placement near the medial epicondyle and the sublime tubercle must respect the course of the ulnar nerve posterior to the medial epicondyle and the flexor-pronator origin that overlies the ligament.
