Description
Oriented in a lateral perspective, the elbow complex is presented with the distal humerus articulating with the radial head and the proximal ulna at the humeroulnar and humeroradial joints. The radial collateral ligament (lateral collateral ligament complex) arises from the region of the lateral epicondyle and spreads distally in a fan-shaped sheet, blending with the annular ligament that encircles the radial head. Deep to the capsular tissues, the ligament fibers sit lateral to the radiocapitellar articulation and anterior to the posterolateral contour of the ulna. Bony landmarks. Clear margins. Clinically, this lateral ligamentous anatomy matters because the radial collateral ligament and its continuity with the annular ligament form the primary restraint to varus stress and posterolateral rotatory instability at the elbow. Tears after a fall on an outstretched hand, or iatrogenic attenuation after lateral epicondylitis surgery, can permit subluxation at the radiocapitellar joint with symptomatic mechanical clicking and a positive pivot-shift test.
