Description
Running along the volar forearm in a medial (ulnar) profile, the flexor digitorum superficialis (FDS) occupies the central mass of the anterior compartment beneath the superficial fascia and skin. Proximally, its humeroulnar and radial heads arise from the medial epicondyle and coronoid process, with fibers also taking origin from the proximal radius, then converge into a broad muscle belly that sits deep to pronator teres, flexor carpi radialis, palmaris longus, and flexor carpi ulnaris. Distally, the FDS tendons course inferiorly toward the wrist, lying superficial to the flexor digitorum profundus (FDP) tendons and passing with them through the carpal tunnel under the flexor retinaculum. The median nerve typically runs deep to the FDS proximally, then becomes more superficial in the distal forearm before entering the tunnel. That central position makes FDS the workhorse landmark when teaching layered flexor anatomy and tendon pathways. Isolated FDS testing (holding adjacent fingers in extension while flexing the proximal interphalangeal joint) helps localize loss of PIP flexion to an FDS tendon laceration rather than an FDP injury, a distinction that drives operative planning in zone II (no man’s land) repairs. It also frames why swelling from flexor tenosynovitis or post-traumatic scarring can increase carpal tunnel pressure, with the FDS tendon synovium contributing to median nerve compression symptoms.
