Description
Arising from the medial perigenicular venous network, the superior medial genicular vein tracks along the superomedial aspect of the knee, coursing deep to the distal vastus medialis and adjacent to the medial femoral condyle. Proximally it ascends toward the adductor canal and converges with deep femoral venous drainage, typically via tributaries to the popliteal vein and the femoral vein. Around the joint line, small venules and anastomotic channels interconnect it with the inferior medial genicular vein and the periarticular plexus near the medial collateral ligament. Orientation is clear. The femur and proximal tibia frame the vessel as it wraps posterior to the medial epicondylar region. Medial genicular veins matter when you are teaching or planning around periarticular venous outflow, because these channels sit in the same operative field as the medial parapatellar arthrotomy, the distal vastus medialis split, and exposure of the medial femoral condyle. During total knee arthroplasty, medial meniscal repair, or MCL procedures, small genicular venous branches are a common source of persistent oozing that can obscure the capsular plane and prolong tourniquet time. Their anastomoses also explain why superficial bruising can track widely after medial knee trauma even when the primary injury is localized.
