The distal femur sits superior to the tibial plateau, and the tibia lies medial to the fibula, with articular cartilage capping the femoral condyles and tibial surfaces. Anteriorly, the patellar tendon courses from the patella toward the tibial tuberosity as a clear landmark for the extensor mechanism. This medial angle matters because the suprapatellar recess is where fluid collects early in a knee effusion, distending above the patella when the quadriceps relaxes.
