Description
Plantar soft tissues are reflected to expose the plantar interossei nestled against the medial aspects of the metatarsal shafts, occupying the intermetatarsal spaces deep to the plantar aponeurosis. Each muscle belly arises from a single metatarsal (classically the third, fourth, and fifth) and courses distally toward the bases of the proximal phalanges, converging on the extensor expansion on the medial side of digits 3 to 5. Nearby, the flexor digitorum longus tendons run more superficially toward the toes, with the lumbricals and interosseous tendons crossing the metatarsophalangeal joints in close proximity. Small muscles. Tight spaces. Functionally, the plantar interossei adduct the toes toward the second ray (PAD) and assist the lumbricals with metatarsophalangeal flexion while extending the interphalangeal joints, a coupling that becomes clinically obvious in claw toe and crossover toe patterns when intrinsic weakness follows diabetic neuropathy or tibial nerve injury. Surgeons working in the intermetatarsal spaces (for Morton neuroma excision, intermetatarsal bursitis, or metatarsalgia procedures) need this plantar perspective because the interossei form the deep muscular floor adjacent to the metatarsals while the plantar digital nerves and vessels travel more superficially and distally. It also clarifies how intrinsic muscle atrophy can destabilize the transverse metatarsal arch and shift load to the metatarsal heads.
