Description
Running along the plantar lateral border of the male foot, the flexor digiti minimi brevis (flexor digiti minimi brevis pedis) lies superficial to the fifth metatarsal shaft and deep to the skin, with its belly oriented longitudinally toward the fifth metatarsophalangeal joint. Proximally, it arises from the plantar surface of the base of the fifth metatarsal and the sheath of the fibularis (peroneus) longus tendon near the cuboid groove. Distally, its tendon passes anteriorly to insert on the lateral aspect of the base of the proximal phalanx of the little toe, sitting plantar to the MTP joint capsule and adjacent to the abductor digiti minimi along the lateral plantar arch. Lateral plantar intrinsic anatomy matters when pain localizes to the bunionette region or the plantar aspect of the fifth ray, because you need to distinguish muscular strain from joint synovitis, sesamoid complex irritation, or fracture at the fifth metatarsal base. The flexor digiti minimi brevis helps stabilize the fifth MTP joint during toe-off, and its relationship to the superficial branch of the lateral plantar nerve makes it a useful landmark when discussing lateral plantar nerve entrapment and postsurgical sensory symptoms after bunionette correction.
