Description
Exposed beneath the plantar skin, the lateral band of the plantar aponeurosis courses from the lateral process of the calcaneal tuberosity toward the fifth ray, forming a thickened fascial strip along the lateral border of the sole. Anteriorly it fans into superficial digital slips that blend with the fibrous flexor sheaths and plantar plates at the metatarsophalangeal joints, while its deep surface maintains close contact with the abductor digiti minimi and the lateral belly of flexor digitorum brevis. Medially, the lateral band is separated from the central band by thinner interposed fascia and the interval that transmits the lateral plantar neurovascular bundle. Lateral heel and plantar border pain is often misattributed to Achilles or peroneal pathology, so a clear plantar view of the lateral band helps you localize symptoms and understand why palpation just distal to the calcaneal tuberosity can reproduce pain from lateral plantar fasciopathy. It also provides a useful anatomic framework for Baxter nerve (first branch of the lateral plantar nerve) entrapment, which occurs deep to the abductor hallucis and along the medial calcaneus but can present with plantar heel pain that radiates laterally. Surgical planning benefits from seeing how limited partial plantar fascia release can spare the central band while addressing focal lateral band thickening or adhesions after trauma. Use this illustration in gross anatomy labs and lower-limb musculoskeletal courses to teach the layered organization of the plantar soft tissues and the relationship of the plantar fascia to the intrinsic muscles.
