Description
Posteriorly, the axis (C2) sits immediately inferior to the atlas (C1), its paired laminae converging medially into a broad bifid spinous process that projects posterior to the vertebral arch. Superior articular facets of C2 face superolaterally to meet the inferior articular facets of C1, while the transverse processes extend laterally and frame the transverse foramina along the cervical column. Inferiorly, the C2 articular processes align with C3, continuing the stacked cervical vertebrae in anatomical position. Small details matter here. Emphasis on the posterior elements makes this perspective useful for correlating bony landmarks with the suboccipital and deep posterior cervical musculature, including attachments near the C2 spinous process that help generate extension and fine rotation at the upper neck. Clinically, C2 is central in trauma patterns such as traumatic spondylolisthesis of the axis (Hangman fracture), where bilateral pars interarticularis fractures occur between the superior and inferior articular processes and can be appreciated relative to the posterior arch anatomy. It is also the vertebra most often chosen for C2 pedicle or pars screw fixation in posterior cervical stabilization, where the relationship of lamina, lateral mass, and transverse foramen (vertebral artery corridor) drives safe trajectory planning.
