Description
Centered in the upper cervical spine, the C2 vertebra (axis) sits immediately inferior to C1 (atlas) and superior to C3, with its dens (odontoid process) projecting superiorly toward the anterior arch of the atlas. Paired superior articular facets lie posterolateral to the dens and meet the atlas, while the inferior articular facets face toward C3 to continue the zygapophyseal joints. Laterally, each transverse process contains a foramen transversarium, and posteriorly the bifid spinous process and laminae frame the vertebral arch around the vertebral foramen. C1 and C2 are color-highlighted against the remaining cervical vertebrae (C3 to C7), which remain in standard bone tone to preserve level-to-level orientation. Rotation of the head occurs primarily at the atlanto-axial complex, and the dens is the mechanical centerpiece, restrained by the transverse ligament of the atlas and flanked by the alar ligaments. Small changes in alignment here matter. Odontoid fractures (Type II at the base of the dens is a classic pattern), atlanto-axial instability in rheumatoid arthritis, and traumatic subluxation can threaten the upper cervical spinal cord and medulla, while the vertebral artery’s course through the transverse foramina sets a boundary for posterior and lateral exposure. This view gives a clean teaching reference for the pivot joint and its bony constraints. Useful for gross anatomy and neuroanatomy courses when introducing craniocervical junction mechanics, and for orthopedic or neurosurgical teaching files covering odontoid fixation strategies and C1 to C2 instrumentation landmarks.
