Spanning the dorsal aspect of the wrist, the extensor retinaculum appears as a transverse fibrous band anchored laterally to the distal radius near the radial styloid and medially to the ulna and ulnar styloid region, bridging over the carpal bones. Deep to it, the extensor tendons course distally toward the metacarpals and phalanges, held close to the dorsal carpal arch rather than bowstringing during finger extension. Beneath the band, the dorsal carpal articulations and outlines of the scaphoid, lunate, triquetrum, and hamate sit proximal to the metacarpal bases, with the digits extended and aligned in anatomical position. Retinacular septa would partition the tendon compartments along the dorsum. Functionally, this posterior perspective matters because the extensor retinaculum defines the six dorsal compartments used daily in imaging and hand surgery, and it explains the predictable patterns of dorsal wrist tenosynovitis. Intersection syndrome occurs where the first compartment tendons (abductor pollicis longus and extensor pollicis brevis) cross over the second compartment tendons (extensor carpi radialis longus and brevis) proximal to the retinaculum, producing pain with resisted wrist extension. At the radial styloid, stenosing tenosynovitis of the first dorsal compartment (De Quervain disease) and ganglion cysts are both interpreted in relation to retinacular constraint and tendon gliding. Small anatomy, big consequences.
