Description
Posterior lower leg anatomy is centered on the fibular (peroneal) artery as it descends in the deep posterior compartment, closely apposed to the medial surface of the fibula and positioned lateral to the posterior tibial artery. Proximally, the vessel is traced back to the popliteal artery behind the knee, with the tibia medial and the fibula lateral forming the bony frame for the neurovascular plane. Distally, the artery continues toward the ankle region, where its perforating and calcaneal branches are implied by its course toward the lateral malleolus and hindfoot. Clear relationships. Understanding the fibular artery matters when mapping collateral supply to the foot and ankle, since it can remain a dominant inflow when the anterior tibial or posterior tibial artery is hypoplastic or occluded. Surgeons planning a fibular free flap depend on this anatomy: the peroneal artery provides the pedicle, and its position against the fibula explains both the reliability of the flap and the need to preserve runoff to the distal limb. A posterior perspective also mirrors common dissection and exposure planes in the deep posterior compartment, where compartment syndrome, traumatic vascular injury, or iatrogenic compromise can be assessed against fixed bony landmarks. Use this illustration in lower-limb anatomy teaching blocks, orthopedic and plastic surgery courseware covering fibula harvest and ankle reconstruction, or vascular surgery and interventional radiology materials discussing infrapopliteal disease and tibial-peroneal trunk variants. It also fits textbooks and patient-facing diagrams that need the relationship of tibia, fibula, popliteal vessels, tibial arteries, and accompanying deep and superficial veins in a single posterior plate.
