Description
Running deep in the posterior compartment of the leg, the fibular (peroneal) artery descends from the tibioperoneal trunk and tracks inferiorly along the medial crest of the fibula, positioned between soleus posteriorly and the deep flexor group, chiefly flexor hallucis longus and tibialis posterior, anteriorly. Distally it stays lateral to the posterior tibial artery while sending perforating branches toward the lateral compartment, and smaller arterioles toward the ankle and calcaneal region. Superficial venous channels course in blue over the gastrocnemius-soleus mass, contrasting the deep arterial pathway. Bilateral lower legs and feet appear in posterior view with the heels and Achilles region clearly establishing orientation. Posterior exposure matters here because the fibular artery is the dominant inflow to the lateral ankle and calcaneus, and its perforators become the surgical lifeline for fibular flap harvest and many lateral leg perforator flaps. Injury patterns in distal fibula fractures and syndesmotic fixation can threaten these branches, while atherosclerotic disease may spare the fibular artery when the anterior or posterior tibial arteries are occluded, making it a key target for distal revascularization planning. A clean posterior pathway also helps explain why compartment syndrome of the deep posterior compartment can jeopardize perfusion without obvious superficial vascular changes. Small vessel anatomy.
