Description
Running in the deep posterior compartment of the leg, the fibular (peroneal) artery descends along the medial aspect of the fibular shaft, typically arising from the posterior tibial artery distal to the popliteal bifurcation. Distally, muscular branches course laterally toward fibularis longus and brevis, while perforating branches reach the anterior compartment near the distal interosseous membrane, where they anastomose with anterior tibial and dorsalis pedis territories. Superficial venous return is contrasted by the great saphenous vein on the medial side of the leg and across the medial malleolar region, with deeper paired venae comitantes implied along the accompanying arteries. Orientation is clear: tibia lies medial, fibula lateral, with vascular pathways mapped in relation to both. Anatomically, the peroneal artery is the “reserve” supply to the foot and ankle, and this course matters when anterior tibial or posterior tibial flow is compromised. In fibular free-flap harvest for mandibular or long-bone reconstruction, surgeons rely on predictable septocutaneous and musculocutaneous perforators from the fibular artery, and injury to these branches can jeopardize skin paddle perfusion. It also enters clinical decision-making for distal bypass planning and endovascular workups in critical limb-threatening ischemia, where peroneal-dominant runoff is common.
