Description

Running from the tibioperoneal trunk, the fibular (peroneal) artery descends in the deep posterior compartment of the male leg, closely applied to the medial crest of the fibula and lying posterior to flexor hallucis longus. Medially, the vessel relates to tibialis posterior and flexor digitorum longus, while the posterior tibial artery and tibial nerve remain more medial in the compartment. Distally, perforating and communicating branches reach toward the lateral malleolar region and anastomose around the ankle and calcaneus with lateral plantar and malleolar networks. Companion venae comitantes track alongside the artery. This medial perspective matters because the fibular artery often becomes the dominant runoff when the anterior tibial or posterior tibial artery is hypoplastic, occluded, or sacrificed, and it can be the key supply to the lateral leg and heel.
