Description
Running along the posterolateral aspect of the leg, the fibular (peroneal) artery is shown arising from the posterior tibial artery in the proximal calf and descending deep to the soleus within the posterior compartment. From an anterior viewpoint, the vessel’s course is understood relative to the fibula laterally and the tibia medially, with the anterior tibial artery passing through the interosseous membrane to the anterior compartment while the fibular artery remains posterior. Distally, perforating branches communicate toward the lateral malleolar region and the dorsum of the foot, and accompanying venae comitantes track closely with the artery; more superficial venous return is represented by the great saphenous vein along the medial leg. Clinically, the fibular artery matters because it often carries the dominant blood supply to the lateral compartment and contributes to collateral flow around the ankle when the anterior or posterior tibial arteries are compromised. Surgeons and interventionalists use this anatomy when planning distal bypass targets, endovascular revascularization for chronic limb threatening ischemia, or flap surgery, since the fibular artery and its perforators support fibular osteocutaneous and lateral leg flaps. Orientation from the front also helps clarify why compartment boundaries and the interosseous membrane affect access, hemorrhage patterns, and pulse findings. Palpation will not find it. Educators can place this artwork in lower limb anatomy and clinical correlations modules to teach the branching pattern from femoral to popliteal to tibial vessels, and to contrast deep arterial pathways with the superficial great saphenous vein.
