Description
Centered on the popliteal fossae of both lower limbs, the inferior medial genicular artery branches from the popliteal artery and tracks medially along the posterior aspect of the knee before curving around the medial side of the proximal tibia (tibial head region). Proximal to it, the femoral artery transitions through the adductor hiatus to become the popliteal artery, which lies deep in the fossa with the popliteal vein typically superficial to it and the tibial nerve most superficial and slightly lateral. Distally, the popliteal vessel continues toward its terminal division into the anterior tibial artery and posterior tibial artery, with paired venae comitantes accompanying the arteries. Posterior anatomy at the knee matters because the genicular branches participate in the periarticular anastomosis that maintains collateral flow during flexion and in the presence of superficial femoral or popliteal occlusive disease. Orthopedic and sports procedures around the medial proximal tibia, including high tibial osteotomy and posterior cruciate ligament reconstruction with tibial tunnel work, place the inferior medial genicular artery at risk due to its close relationship to the posteromedial capsule and adjacent bone. Small vessel, big consequences. This posterior perspective also aligns with the operative corridor used for popliteal artery exposure in vascular repair after knee dislocation.
