Description
Presented in anterior anatomical position, both lower limbs are rendered with the arterial tree in red and the venous return in blue, centering attention on the inferior medial genicular artery as a branch arising from the popliteal artery near the knee. From its origin in the popliteal fossa, the vessel courses anteromedially around the proximal tibia, forming a characteristic curve as it passes deep to the medial tibial plateau and toward the medial aspect of the knee joint. Adjacent landmarks you would expect in this relationship include the medial femoral condyle superiorly, the tibial tuberosity and anterior tibial surface anteriorly, and the course of the great saphenous vein more superficially along the medial leg. Small-caliber anastomotic branches are implied around the joint line. A tight corridor. Understanding the inferior medial genicular artery matters because it contributes to the periarticular genicular anastomosis and to vascular supply of the medial knee capsule, adjacent periosteum, and proximal tibia. This is the vessel surgeons learn to respect during medial approaches to the proximal tibia and knee (including high tibial osteotomy, medial meniscal repair portals, and exposure for tibial plateau fracture fixation), where unchecked injury can lead to troublesome bleeding or postoperative hemarthrosis. Its position under the medial tibial plateau also explains why the artery can be a source in periarticular bleeding and why targeted genicular artery embolization has been explored for refractory knee pain or recurrent hemarthrosis in select cases. Use this plate in lower-limb anatomy teaching to connect the popliteal artery to the genicular network, or in orthopedic and sports-medicine publications discussing medial knee approaches and vascular risk zones.
