Description
Arising along the lesser curvature of the stomach, the left gastric vein ascends toward the gastroesophageal junction and tracks superiorly to the region of the abdominal esophagus. From a posterior perspective, its course is appreciated behind the proximal stomach as it converges near the cardia, where venous tributaries from the anterior and posterior gastric walls gather before draining toward the portal venous system. The fundus lies superior and leftward, while the pyloric region sits inferior and to the right, helping orient the vessel’s proximal to distal relationships along the stomach’s J-shaped contour. Blue vascular channels on the gastric surface cue the venous network that parallels the left gastric artery in the lesser omentum. Posterior visualization of the left gastric vein matters because this vessel forms a key portosystemic connection at the distal esophagus via anastomoses with esophageal veins that communicate with the azygos system. Portal hypertension can redirect flow here, producing esophageal varices at the gastroesophageal junction, a common site of life-threatening upper gastrointestinal hemorrhage and a frequent target for endoscopic band ligation. During proximal gastric surgery or esophagogastrectomy, recognizing the left gastric vascular pedicle and its venous drainage helps frame lymphadenectomy around the left gastric vessels and reduces the risk of bleeding in the lesser curvature region.
