Description
Presented from the posterior pelvis into the proximal thigh, the sciatic (ischiadic) nerve forms a thick cord emerging inferior to the piriformis and deep to gluteus maximus before descending along the posterior compartment of the thigh. Its course runs lateral to the ischial tuberosity and posterior to the adductor magnus, with the hamstring muscles (biceps femoris laterally, semitendinosus and semimembranosus medially) framing it as it travels distally. Proximal branches to the posterior thigh are suggested along the nerve’s surface, and the typical bifurcation into tibial and common fibular (peroneal) components is indicated as the trunk approaches the popliteal region. Orientation is straightforward. Posterior anatomy matters here because the sciatic nerve is a frequent target in deep gluteal pain syndromes and in iatrogenic injury from misplaced intramuscular injections. Seeing the relationship to the piriformis and the short external rotators helps explain why piriformis variant anatomy can alter symptoms and why a posterior hip approach, or repair of proximal hamstring avulsion at the ischial tuberosity, demands careful identification and protection of the nerve. The tibial and common fibular division also sets up the clinical patterning of foot drop versus plantarflexion weakness in lesions around the popliteal fossa or fibular neck.
